The Senior Community Service Employment Program (SCSEP), also known as the Title V Program, is a federally funded program designed to assist adults age 55 and older in entering or reentering the job market.The program is administered by the Illinois. The Turner Senior Wellness Program is proud to be part of the Geriatrics Center at Michigan Medicine, which provides substantial support. Yet to cover the full cost of these community services, we rely on. GEM-FIT (Geriatric Emergency Management - Falls Intervention Team) Seniors - elderly - deteriorating health or isolation in the community. Elderly - parents - neighbour - friends - health is deteriorating.
Description:
The Community Care for the Elderly (CCE) Program provides community-based services organized in a continuum of care to help functionally impaired elders live in the least restrictive yet most cost-effective environment suitable to their needs.
Services or Activities:
Eligible clients may receive a wide range of goods and services, including: adult day care, adult day health care, case management, case aide, chore, companionship, consumable medical supplies, counseling, escort, emergency alert response, emergency home repair, home-delivered meals, home health aide, homemaker, home nursing, information and referral, legal assistance, material aid, medical therapeutic services, personal care, respite, shopping assistance, transportation, and other community-based services.
Administration:
The Department administers the program through contracts with Area Agencies on Aging, which subcontract with Community Care for the Elderly lead agencies. Service delivery is provided by 52 lead agencies and their subcontractors.
Eligibility:

Individuals must be age 60 or older and functionally impaired, as determined by an initial comprehensive assessment and annual reassessments. As directed by 1998 revisions to Section 430.205(5), Florida Statutes, primary consideration for services is given to elderly persons referred to Adult Protective Services (APS) and determined by APS to be victims of abuse, neglect, or exploitation who are in need of immediate services to prevent further harm.For more information about CCE in your area, contact your local Aging Resource Center or call the Elder Helpline at 1-800-96-ELDER (1-800-963-5337)
